Labradors and Friends Dog Rescue Group Adoption Application
 
Thank you for your interest in adopting a dog from Labradors and Friends Dog Rescue Group! As a rescue organization, we take great care in saving these dogs from euthanasia and finding them new homes. In order to facilitate the best match between dog and family, we require the below application and a home check visit be completed prior to adopting a dog from us.

While we try to be prompt in response, we are an volunteer organization, so please allow at least 48-72 hours for a response. Of course, you can always email us at labradorsandfriends@yahoo.com to ensure your application is in process. Thank you for considering a rescue dog!

IN ORDER TO BE CONSIDERED AS AN ADOPTER TODAY, YOU MUST:




Be 18 years of age or older

Have identification showing your present address

Have the written consent of your landlord (if you rent)

Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care throughout the entire life of the dog.

The responses you provide on this application will help us to find the best possible match between you and the dogs available through our organization. Please include area codes with all phone numbers. If no phone numbers are given, this application will not be considered. Please respond to questions below within space constraints given. Once we receive your application, you will be contacted by one of our volunteers to discuss your application and schedule a home check visit. Home checks are required for all applicants so we can ensure our dogs go to a safe and loving home. Labradors and Friends Dog Rescue Group reserves the right to deny an application for any reason.

Labradors and Friends Dog Rescue Group is an all-volunteer organization. Please allow sufficient time for an LFDRG Representative to contact you.

General Agreement

I am at least 18 years old. All of the information I have given above is true and complete. Should an unaltered dog be placed with me, I agree to have it neutered within one month of adoption. This dog will reside in my home as a pet and will not be strictly an outdoor pet. I will provide it with adequate food, water, shelter, training, affection, and medical care. I understand that Labradors and Friends Dog Rescue Group is not responsible for the accuracy of information received about the temperament, habits, or physical conditions of dogs available for adoption. I understand that it is my responsibility to see and evaluate the dog myself before agreeing to adopt it. I will not relinquish ownership, abandon, or dispose of such dog in any way. If I cannot keep him or her, I will turn the dog over to Labrador and Friends Dog Rescue Group I do not expect reimbursement if this happens. I am in full agreement with these terms of adoption. Labradors and Friends Dog Rescue Group is not liable for any damage, accident, or injury resulting from the placement of a dog into my household.
