By submitting this application, I confirm under the penalty of perjury that I am at least 18 years old and that all of the above information I have given is true, correct and complete based on my personal knowledge.
I understand that in addition to the on-line submission of this application, the Labradors and Friends Dog Rescue Group Inc., (“LFDRG”) application review process requires submission of various personal documents and third party verification of such documents, photographs, interviews, physical and/or virtual property inspections, signed contractual agreements and in the case of an adoption, payment of the adoption fees listed on the website.
My submission of this on-line application does not create any duty or obligation on the part of LFDRG to place any dog in my home. I specifically acknowledge that LFDRG may in its sole and unlimited discretion decline my application in its entirety or as to a specific dog for any reason or for no reason whatsoever.
Any dog placed in my home will reside in my home as primarily an indoor dog. I will provide such dog with adequate and appropriate food, water, shelter, training, socialization, affection, and medical care as such dog requires.
In the even I adopt a dog under this application, I acknowledge that I am able and willing to spend the time and money necessary to provide adequate and appropriate food, water, shelter, training, socialization, affection, medical treatment, and proper care throughout the entire life of the dog. I further acknowledge that any unaltered dog adopted by me, must be altered within 90 days of adoption, or sooner should LFDRG request an earlier alteration date.
In the even I foster a dog under this adoption application, I acknowledge all references to “dog” in this on-line application and any related contractual agreement include any dog fostered by me. I understand adoption fees are not waived for adoptions by the foster of his/her foster dog or any dog for that matter.
I understand that LFDRG is not responsible for the accuracy of information received about the temperament, behavior, habits, socialization, physical conditions or medical conditions of dogs available for fostering or adoption. I understand that it is my duty and my sole responsibility to see and evaluate the dog myself before agreeing to foster such dog or requesting to adopt such dog.
I will not relinquish, abandon, or dispose of any dog placed in my home in any way. If I cannot keep such dog, I will return the dog to LFDRG. I understand I will not be reimbursement for any expenses (transportation related or otherwise) if this happens.
I fully understand and agree to assume any and all risks (known or unknown) involved in any actions, tasks or duties I may perform or undertake during the application approval process, any dog fostering, any dog adoption and any dog ownership arising out of such process. I further agree to indemnify and insure and hold LFDRG harmless from any and all claims arising out of my application to adopt a dog, any activity undertaken to further the approval of that application, and any resulting dog fostering, dog adoption and/or ownership of a LFDRG Dog, including but not limited to: third party claims brought against me, my family, LFDRG and/or its agents, assignees and/or successors in interest; and to any claims related to injury(s) of any nature which I, my family members, personal property and/or family pets may sustain.
